I was brought up to rarely see a doctor. Blimey, if I had to count the number of times I've used a hospital in over 50 years, it'll be over 10 but less than 20. On the other hand, the Mrs makes up for my lack of attendance.
So should she contribute a small amount each time? Even if it was just £1 per visit, it's affordable. With millions of patient visits per year, the £1 contributions will soon add up.
If you miss a doctors appointment, should you be charged? You don't have to pay the charge but a doctor can only see you if you have no outstanding invoices. Around 7.8 million are missed each year. So charge £10 for a missed appointment, £70m can go towards training doctors and nurses.
In other words, make it a little more PAYG.
